Every Student-Athlete must have a current Athletic Physical on file with the Athletic Office (completed after April 15 of the prior year) in order to be eligible to tryout or practice. Please download both pages of the Physical by clicking on the link below:
Any students who received a valid sports physical during the 2019-20 school year (one completed on or after April 15, 2019), may complete the 2020-21 MHSAA SPORTS HEALTH QUESTIONNAIRE and it will be valid for the upcoming school year. Any student who did not have a valid physical, or is new to middle school and high school sports, will be required to have a physical exam from a MD, DO, NP, PA prior to participation using the MHSAA Sports Physical Form.

*In order to participate in 2020 summer workouts, we are requiring each athlete to turn in either a completed 2020-21 MHSAA Health Questionnaire or an updated MHSAA Physical form. The forms are available online or in the athletic office.
Please note, there is an assumption of risk when participating in MHSAA-sponsored athletics, therefore we are also requiring both athletes and coaches to sign the Covid-19 waiver. Coaching staff will hand these out to athletes/parents or you can download the form here:
